Total Hip Replacement

September 26, 2013 :

This patient has previously had a fracture of the hip in which the surgeon was operated elsewhere and a dynamic hip screw implant was used. However, this implant failed on the patient causing infection and a result a non union fracture with broken pins. A Uncemented Total Hip Replacement via transtrochanteric approach procedure was done. The patient has recovered well.

A 71 year old male had a road accident fracturing his left acetabulum as well as sustaining a transverse fracture of the posterior wall. 3D CT reconstruct and judet views showed break in anterior column, posterior column as well as the posterior wall fracture and a dislocated femoral head. Open Reduction and Internal Fixation through Kocher Langen Back Approach was done. Posterior wall was fixed with 3 compression screws and posterior column reconstructed over the wall. Head was reduced back in place. Anterior column reduction was done indirectly using a collinear clamp.

Knee Replacement of a 63 year old lady had been suffering from pain in both the knees for the last 8 years. She also had difficulty in walking because of progressive deformity in both the knees. She could walk only few steps with the help of walker.
X-rays showed osteoarthritis of both the knees with severe varus deformity and bone loss of proximal tibia. In this patient, we managed using conventional implants along with bone grafting and screw fixation with a stem on the tibial side.

A 63 year old lady was suffering from pain in the right knee for the last 15 yrs. She was almost bedridden for the last 4 years because of progressive deformity of her right leg. Her right knee was bowing outwards. As a result her knees were knocking against each other and her legs were now of different length. She was found to be having osteoarthritis of the right knee resulting in valgusdeformity. Majority (> 95%) of the patients have a varus deformity in which the leg bends inwards; only a few patients have valgus deformity in which the leg bends outwards. In this patient we could manage a good result using conventional implants. However, there are occasions when we have had to use special Constrained Condylar implants for the knee. She is doing very well in the recovery period.

Mr JK, a 54 year old gentleman had complaints of pain and progressive bending of both the knees for the last 12 years. He was completely housebound for the last 2 years due to difficulty in walking. He had no other medical problems. His x-rays showed that bone from one side of tibia had completely worn out. This was a patient who should have undergone total knee replacement at least 6 years back. We had to use a special part called a Wedge. This is made of metal and it fills up the bone deficiency. The patient was out of bed on the 3rd day of surgery and is recovering well.
