Infected 2nd Stage Revision Total Knee Replacement (RHK)

A 70 year old diabetic and hypertensive gentleman underwent B/L Total Knee Replacement(TKR) 2 years back. He developed pain andswelling in Right knee. For which aspiration of knee was done which confirmed infection. Initially nonoperative treatment with long term suppressive antibiotics was done which failed. Thereafter we planned for2 stage revision Total Knee Replacement(TKR).

Management of infected Total Knee Replacement(TKR) is challenging and very few centers in the country are routinely doing these cases. We at our center follow a 2 stage protocol for these cases. In the 1st stage n infected prosthesis is removed along with thorough debridement and antibiotic mixed cement spacer is placed. In the 2nd stage the spacer is removed and a revision prosthesis Right Hip Replacement(RHK) is implanted.At 2 weeks post-operatively the patient is walking comfortably with a frame.

Total hip Replacement (THR)-DEC-2015:
RNG 66 M presented to us with post traumatic arthritis of left hip . He sustained fracture acetabulum following road traffic accident which was treated non operatively . He complained of inability to bear a weight on left side lower limb due to severe pain and was bedridden . We performed uncemented Total hip Replacement (THR). THR in acetabular fracture is technically demanding and high level of surgical expertise is require for performing this surgery. Post operatively patient is happy and ambulatory with the help of walker.

Revision total knee replacement from Aseptic loosening

70 years old lady had total knee replacement on both sides 15 year back. Though she was carrying on well with her left side, her right knee started paining for last 1 year. Serial x-rays and blood test of the affected side showed non infected loosening of previously implanted components.

Treatment

Revision total knee replacement was performed on left knee. Previous loose parts were removed and special new revision implants inserted. The patient was started on physiotherapy and is ambulated with walking frame.

Total HIP Replacement

28 years female presented to us with pathological fracture of right hip. She had aggressive tumor ( Giant Cell tumor) of the proximal femur for which curettage was done 3 months back. Now she was completely bed ridden because of the fracture and recurrence of tumor. Previously there was very limited options for treatment of such patients and most the patients remain bed ridden for rest of his life. With the evolution of modern orthopaedics and surgical expertise we were able to take out the tumor and replace the lost bone with the metallic part. Posopertavely patient is comfortable and out of her bed.

Total Hip Replacement for Post Traumatic Arthritis of the hip.

70 years old gentleman presented to us with pain in the left hip since last 6 months . He had a hip fracture 2 yrs back which was operated (CRIF with DHS) . He was not able to load his left lower limb and his activities were severely restricted. At present he was household ambulator with support.
We have removed the previous implant and done uncemented Total hip Replacement with long femoral stem. Postoperatively patient is comfortable and walking independently.
